Hours Passport Fees

Passport services are now available:

Monday - Friday

8am -12pm and 1pm - 3pm

Closed: from 8am -10am on Thursdays

Lincoln County Health Department Fees: 

  • Passport Photos $15

  • Expedite Shipping $30(optional)

  • Copy Services $1/page 

  • Acceptance Fee per Passport Application $35 

Acceptable forms of payment to the Lincoln County Health Department include Visa, Mastercard, check and cash. 

 
 
 

how to apply

Prior to applying for a passport you will need:

  • An application completed in black ink

    • Applications are available in the LCHD reception area, or online

  • A form of U.S. Citizenship 

    • Birth Certificate or previous Passport

  • A form of Identification 

    • Valid Drivers License

  • A check or money order to be mailed with your application
