Our Community Health Worker (CHW) has helped community members do many different things. They can help you:

  • Find financial help

  • Find a primary care doctor

  • Get connected with food help

  • Fill out job applications

  • Make a doctor's appointment

How to get the process started

The 1st step is reaching out to our office and asking for our Community Health Worker (CHW). Next, our CHW will call you to talk about your situation. You will work together to meet your needs.

We can help either in person or over the phone. These services are available to anyone and are free. Your information is always private.

Sometimes all you need is a helping hand.

Many of us are facing challenges that can be stressful and overwhelming. This could include:

  • Losing your job

  • Losing your health insurance

  • Getting diagnosed with new health conditions

  • Loss of family or friends

  • Money worries and struggles

  • Mental health problems
