Birth and Death Certificates are printed on-site by request and available Monday through Friday from 8:00 AM until 4:30 PM, except for observed holidays.
Identification is required for any person requesting a vital record. This should be a document containing a photo, such as a driver’s license, passport, or school, work or military identification. If a photo ID is not available, at least two alternate forms of identification must be presented. The documents must include the applicant’s name and the name of the company or organization issuing the document.
How to obtain Vital Records:
To obtain a vital record in person you will need to fill out and sign the application, provide proof of identity and possibly tangible interest documents or a signed notarized statement authorizing the release. If you obtaining a vital record for a family member, additional documentation may be needed. Those who are directly related to whom you are obtaining a record for may receive a copy of the vital record.
Death Certificates:
$14.00 for first copy; $11.00 for each additional copy requested at same visit
For persons who died in the state of Missouri from 1980 until present
Birth Certificates:
$15.00 per certificate
For persons born in the state of Missouri from 1920 until present
Records may also be requested by mail by sending a notarized copy of the certificate form and check to:
