Passport FAQ
I’m 18 or older applying for a passport for the 1st time, what do I need to do?
- Fill out form DS-11. You can download it for free online or pick it up for free at the Lincoln County Health Department lobby during business hours. 
- Bring evidence of citizenship. This could be: - State-issued birth certificate 
- Certificate of birth abroad 
- Certificate of naturalization 
- If you cannot submit one of the above listed documents, see more information on citizenship evidence 
 
- Bring photo identification, such as: - Valid MO driver’s license 
- Government employee ID (city, county, state, federal) 
- US military/dependent ID 
- Current foreign passport 
- If you do not have one of these photo ID’s, see more information on identification evidence 
 
- Provide a photo - Lincoln County Health Department will take your photo for $15.00 each

- Calculate the fees you will pay. You will pay 2 separate fees. - US Department of State fees: Must pay exact amount by check or money order per application - Adult Passport Book: $130 Minor Passport Book $100 
- Adult Passport Card: $30 Minor Passport Card $15 
- Adult Passport Card and Book: $160 Minor Passport Card and Book $115 
- Add $60 to you fee if you want expedited service 
- Add $19.53 to your fee if you want your completed passport shipped via USPS priority mail express (1-2 day delivery) 
 
- Lincoln County Health Department fees: Any form of payment is accepted - Execution fee (per DS-11 form): $35 
- Photo fee: $15 
- Priority Mail Express $30 
 
 
- Make your appointment at the Lincoln County Health Department by clicking here or by calling (636) 528-6117 option 4. - If traveling in the next 2 weeks, you must make an appointment to apply at a federal passport agency
