Passport FAQ

I’m 18 or older applying for a passport for the 1st time, what do I need to do?

  1. Fill out form DS-11. You can download it for free online or pick it up for free at the Lincoln County Health Department lobby during business hours.

  2. Bring evidence of citizenship. This could be:

    1. State-issued birth certificate

    2. Certificate of birth abroad

    3. Certificate of naturalization

    4. If you cannot submit one of the above listed documents, see more information on citizenship evidence 

  3. Bring photo identification, such as:

    1. Valid MO driver’s license

    2. Government employee ID (city, county, state, federal)

    3. US military/dependent ID

    4. Current foreign passport

    5. If you do not have one of these photo ID’s, see more information on identification evidence 

  4. Provide a photo

    1. Lincoln County Health Department will take your photo for $15.00 each 

    2. See photo requirements and examples of photos 

  5. Calculate the fees you will pay. You will pay 2 separate fees.

    1. US Department of State fees: Must pay exact amount by check or money order per application 

      1. Adult Passport Book: $130 Minor Passport Book $100

      2. Adult Passport Card: $30 Minor Passport Card $15

      3. Adult Passport Card and Book: $160 Minor Passport Card and Book $115

      4. Add $60 to you fee if you want expedited service 

      5. Add $19.53 to your fee if you want your completed passport shipped via USPS priority mail express (1-2 day delivery) 

    2. Lincoln County Health Department fees: Any form of payment is accepted 

      1. Execution fee (per DS-11 form): $35

      2. Photo fee: $15

      3. Priority Mail Express $30
